Starting January 1, 2024, dental coverage will be provided through Unum
Dental benefits have been enhanced as both the base and buy-up plans will now include a benefits rollover feature
Maximum Annual benefits on the buy-up plan have increased from $2,000 to $5,000!
Dental Benefit Options
Employees have a choice between base and buy-up options. Both options include benefits for annual cleanings, as well as minor and major restorative procedures. The buy-up plan also includes adult and child orthodontia benefits and provides annual maximum benefits of $5,000. Both plans:
- Cover preventive care at no cost to you
- Pay 80% for Basic Restorative services after deductible
- Pay 50% for Major Restorative services after deductible
- Have a single deductible of $50, not to exceed $150 per family. The deductible does not apply to preventive services
- Use the Unum national dental PPO network
Plan Options
While the Unum Dental PPO network includes thousands of participating dentists, the plan has been structured to minimize the possibility of members being balance billed by out-of-network dentists.
For services provided by a non-network dentist, Unum will reimburse the provider based on the 90th percentile of all provider charges in that area. This high level of reimbursement means that even if your dentist does not participate in the Unum network, the dentist is unlikely to bill you any more than you would pay at an in-network dentist.
