Vision Benefit Options
Vision coverage is offered through Unum, in partnership with EyeMed. The EyeMed Insight network is one of the largest in the country, providing access to more vision providers than ever previously available to Community Brands employees! EyeMed includes independent, national and regional vision providers such as Lens Crafters, Pearle Vision, Target Optical, and many more.
Employees may choose between a Low and High plan. Both plans include benefits for annual eye exams, lenses, frames and contact lenses in lieu of lenses and frames. On the Low plan, most services are subject to a copay, while copays are not required on the High plan. In addition, the High plan allows one to get new frames every year, while the Low plan allows for new frames once every 24 months.
Importance of choosing an in-network provider
Vision benefits for out-of-network providers are substantially lower than in-network benefits. For this reason, covered members are strongly encouraged to use a provider who participates in the national EyeMed network.
